VMM: Call EPT ops for every KPT op

At this point, the EPT should equal the KPT, up to UVPT.

I disconnected the EPT fault handler for now.  tests/vmmcp still works,
since everything is pre-faulted in.  We'll need to change it to do the
initial faults from the VM.
